1. I understand that this treatment is for cosmetic enhancement purposes only and that no guarantees have been made to me regarding the results including fading
2. I am responsible for the "at home after care" which may have risk of infection or fading of pigments if not carried out fully
3. I will not hold the therapist responsible in the event of any damage and shall not be entitled to take action against her at Law and Equity for such treatment
4. I am aware that I cannot give blood for 6 months (this is a universal law)
5. I have had the opportunity to ask questions relating to this treatment
6. I am aware that more than 1 treatment may be necessary for best results, and I am aware that cosmetic tattooing will take at least 2 or 3 procedures to gain best results and that there is a cost for your perfecting session follow up treatments: at an extra cost
7. I understand that there are no refunds given
